In an effort to maintain an safe and healthy environment, any person entering a RevHealth facility is required to sign the COVID-19 FORM. This form must be filled out and submitted each day you enter the facility. This form is required by any person entering our building. Employees, Clients, Guests or Family members. The forms are located at the front desk of each facility and must be handed directly to our receptionists before you can proceed beyond the lobby.
